Residents of Sangamon and Menard counties are asked to give feedback on the community’s health needs.
Springfield Memorial Hospital, HSHS St. John’s Hospital and the Sangamon County Health Department are conducting the Community Health Survey this month.
The survey asks about local health challenges, priorities, and quality-of-life concerns. Previous survey results have led to mental health advocate training and a social care referral program.
